What Is Case Management?
At Halifax Regional Medical Center, the Case Management department is part of a collaborative multidisciplinary hospital-wide process that assesses your healthcare needs and assists you with the development of a plan of care specifically for you. A member of the case management team will evaluate your options and help you determine which of our services or community resources will offer you and your family the highest quality, most cost-effective care.
Think of your case management team member as your personal advocate. They are available for you for the following services:
- Act as a referral source, information source and liaison for patients, families, physicians and other members of the multidisciplinary team
- Provide psychosocial assessment and intervention when appropriate
- Help patients when trying to make difficult end-of-life decisions by providing information and resources for living wills and power of attorney for healthcare.
